Pololu 12V, 100:1 Gear Motor w/ 64 CPR Encoder

I have a couple of Pololu 12V, 100:1 Gear Motor w/ 64 CPR Encoder motors.
I can’t seem to find any reference material that lists the pinout or usage for the encoder.
Most of the other gear motors with encoders come with that info, and they differ enough for me to not want to assume that any of them also apply to my motors.

Is there any information on the pinout/usage for this motor?
