Analog 0-5VDC to 100hz PWM Module/Controller

Hey all,
I am new here so bare with me. I am looking for an off the shelf module/ controller that will suit my needs.
Maybe someone can point me in the right direction.

The end goal is to generate a PWM signal that will drive a brushless dc fan motor that already has an onboard speed controller with a PWM input. The module I need will monitor one or two 0-5VDC inputs and output a PWM based on a pressure transducer and a liquid temp transducer.
Here is a list of the characteristics that I am after.

Supply power:
5 or 12VDC +- 2%

Input(s):
-1x or 2x 0-5VDC Analogue.
Output:
-100Hz, Pulse ratio=10%-95%, PWM signal. (Signal will be switching to ground)
Adjustability:
-Gain control for each inputs proportion, wether it be 1 or 2.

Welcome to the forum.

Not sure if that’s what you are looking for but you can control the PWM of an arduino using a potentiometer on an analog Input.
